Passport Help

Our office will do everything we can to help you obtain your passport; however, we cannot guarantee that you will receive your passport in time for your scheduled departure. Failure to complete the steps below will delay our ability to assist you before your intended travel date.

1. Please fill out the Digital Privacy Release Form. If the individual in need of assistance is a minor, complete the form with the minor’s information (including Social Security Number) and have a parent or legal guardian sign the form. If there are multiple family members in need of assistance, please submit only one Digital Privacy Release Form and, in the area provided to describe your request, include the names of all individuals in need of assistance. The casework team will provide further guidance.
2. Please attach a copy of your flight itinerary or proof of international travel to the Digital Privacy Release Form.
